In a large bowl, combine ground beef, bread crumbs, milk, egg, chopped onion, minced garlic, salt, and pepper. Mix gently with your hands until just combined. Do not overmix.

In a small bowl, whisk together the ketchup, brown sugar, and apple cider vinegar for the glaze.

Bake for 45 minutes. Remove from oven and spread the remaining glaze over the meatloaf.
