Cook pasta according to package directions. Reserve about 1 cup of pasta water before draining.
While the pasta is cooking, heat olive oil in a large skillet over medium heat. Add garlic and cook until fragrant, about 1 minute.
Stir in tomato paste and cook for 2-3 minutes, stirring constantly, to caramelize slightly. This adds depth of flavor.
Pour in heavy cream and bring to a simmer. Reduce heat to low and stir in Parmesan cheese and red pepper flakes (if using).
Season with salt and pepper to taste. If the sauce is too thick, add a little bit of the reserved pasta water until it reaches your desired consistency.
Add the cooked pasta to the sauce and toss to coat. Serve immediately, garnished with fresh basil.
