Place chicken breasts in the bottom of a crock-pot.
In a saucepan, melt butter over medium heat. Add garlic and cook for 1 minute, until fragrant.
Stir in heavy cream, Parmesan cheese, Italian seasoning, salt, and pepper. Bring to a simmer, stirring constantly, until the cheese is melted and the sauce is smooth.
Pour the sauce over the chicken in the crock-pot.
Cook on low for 4-6 hours, or on high for 2-3 hours, or until the chicken is cooked through.
Shred the chicken with two forks.
Cook pasta according to package directions. Drain and add to the crock-pot with the chicken and sauce. Stir to combine.
Serve immediately and garnish with extra parmesan cheese and chopped parsley if desired.
